Telehealth consent
Draft for attorney review
This is a working draft written for Next You Health. It is not final and must be reviewed by a Florida healthcare attorney before launch. The reference site's own policy pages were empty, so nothing here was copied. Last edited November 2, 2026.
Before your first visit, you'll be asked to review and accept this consent inside the clinical platform. It's shown here so you can read it in advance.
What telehealth is
Telehealth means you're evaluated by a licensed clinician using secure messaging, questionnaires, photos or live video instead of an in-person visit. Your clinician decides which method is needed for your treatment, and may require a live video visit.
Benefits and limits
Telehealth can be convenient, but a clinician can't perform a physical exam remotely. If they decide you need in-person care, they'll tell you, and you may need to see a local provider.
Your responsibilities
- Give complete and accurate health information.
- Be physically in Florida during each visit.
- Complete required labs and follow-ups.
- Seek emergency care by calling 911 when needed.
Privacy
Your visit happens through a HIPAA-compliant platform. You can withdraw consent at any time, which ends telehealth care with us.
Items for counsel
Confirm against Florida §456.47 and the clinical platform's own consent; add controlled-substance language for testosterone programs.
